Lydia,

I ordered it from a web site that deals in finding out of print books.  They
located just the one copy.  I made a copy of the entire book and then sold it
to Willem.  It is on its way to Holland as we speak.  Would you like a copy?  I
am offering copies to those interested for $10.00.  I'm using my lunch time to
make copies from the master copy (double sided) and I'll send it out to those
interested (US only).  Willem is taking care of those in Europe that are
interested.
