I generally don't worry too much about it too much.  I guess with the lenses I
have that do not have focusing rings.  I generally stop down, which offers a
greater field of depth.  After focusing, I advance the lens "slightly".

The scans, unfortunately, are not as good as they could be.  Sometimes I blow
out the highlights, usually not, depending upon what I'm trying to accomplish.

Exposure is not a constant, but something that gets changed depending upon the
scene.  "Usually", I take a reading through the filter at EI 250 and fire away. 
Like I said, though, this gets changed all the time, but works for me as a
starting point.  This is one of the problems with roll film, as it has to get
developed with all of the other images on the roll, and why I prefer the large
format when it is reasonable.
