Applied Depth of Field by Alfred A Blaker is about the best book on depth of field.
It covers just about every thing. From 35mm to large format exp depth of field in photography people and scenes,epth of field in near object pnotography,
depth of fielc in closeups and photomacrography,pages of tabels,formulas
and one very great thing easy to use pocket calculater math it shows you how to set up the formulas on the pocket calculater.I have one with a memory I set up the formula punch in focal leanth and f stop and bang there it is.

I'm pretty sure there's a formula for it, I'm trying to track it down...  
although it's not as simple as you might be expecting... it depends on what  
you call "out of focus"
