The mechanism of the Sputnik makes precise vertical positioning of 
the lenses nearly impossible.  Slight movement of the lens is how the 
shutter synch is adjusted.  That's just how the thing was designed. 
I've never considered it a problem, many cameras do not have exactly 
aligned lenses.  I have always used a different mounting method which 
makes slight vertical lens differences immaterial.

No, no.  Built-in trouble isn't exactly right.  Occasionally quirky 
is better.  But if you use a Realist, how can you complain about 
quirky? :^)

Unless it has been badly abused (uncommon), a Spud should work pretty 
well.  Not bad at all for a cheap foreign antique made half a century 
ago that will routinely produce pictures that will blow a Realist 
away.
