  Could the focusing gears be slipping?

  When I got my Sputnik the right lens would
sometimes slip relative to the finder due to
too much play and worn aluminum teeth on the
lenses.

  So I had to loosen up the RH lens and move
it up and in towards the VF lens. I also had
to recalibrate the focus on the two lenses
relative to the finder.
