I got some soft poly pressure caps from a plumbing
store (in a lovely shade of blue). I carved a hole
so that they would friction fit over the lens barrel.
Then I cut the sidewall down to be quite short (1/4" or
so). The threaded part of the polarizer fits inside the
pressure cap and is glued in with silicon sealant.

No, not connected.

They are nice and small. Just a bit over
an inch in diameter. And they have the orientation
dots so it's easy to get both set the same.
I haven't used them much so I'm not an expert
but they worked fine when I did.
