O.K., here's a plain-text version of the NSA registration form for
those who asked.  This should print out o.k., but if not I won't accept
any blame.  I'll follow with a post of the Trade Show Table
Application.

registered adult.  Applications received after July 26, 1996 will
require an additional $10. This year we are offering a choice of three

This has all the info that the mailable form has, so it should suffice. 
You don't get the graphics and nice formatting, but you save a couple 
of stamps!
Note that Trade Show Table applications and Convention Registrations
are returned to different addresses.  You also might want to put your
familiar name in parentheses if you are registering under a formal name
(Bill vs. William, for example) so badges will be a little more
user-friendly.
