How much are you willing to spend?  If you want an "integrated" camera,
several of the RBT cameras do this (e.g. the RBT X2B takes 18 full-frame
stereo pairs on a 36-exp. roll with a 75mm stereo base).  They are
beautifully crafted from existing monocular SLRs.  However, they
are very expensive.

You might also consider twinned SLRs, twinned point-'n-shoots, etc.
There have been several construction articles posted to this list.
