BOLEX STEREO PRESENTATION BOX SET

I recently obtained the 16mm BOLEX STEREO SYSTEM in presentation box. Has a
past Photo-3d digest had any information about the details of this system
and the entire contents (and their usage) of this set which fit on the Bolex
H-16 16mm movie camera?  Here is a list of the items in the box. (Please
forgive the descriptions as I did not get the instructions manual with it.)

I'm putting a question mark in front of the items which I don't yet know the
purpose of...

1  camera attachment w/close up macro dial
2  viewfinder extension metal bracket 
3  two lens sunshade hoods (for screwing onto the camera attachment)
    (Items 4 through 9 are in a small black storage box which fits in
presentation box)
4  (?) one screw dial silver color with black on top and red circle
5  (?) one spring loaded clasp (silver metal & black metal with hole in center)
6  (?) round metal peice with a rectangle in the bottom
7  (?) small mask with words stereo
8  (?) small mask with words stereo and 16 (Right?)
9  (?) another small mask with words stereo and 16 (Left?)
10 (?) stereo (twin window) mask YVar 1:2.8 f=12.5mm Stereo with round
holes & 8 small screw sized holes (The silver metal lens covers fit this
item)
11 (?) stereo (twin window) mask same size as above mask except with
squared holes & 8 small screw sized holes
12 projector lens with S1 sleeve in round storage case which fits in
presentation box
13 8 tiny screws (fits the two loose stereo twin window masks)
14 (?) small silver dial with ribbed edges and two tiny holes with twin
screws &      an outer silver peice with numbers 1, 1.5, 2, 5, 8, & m
15 Bolex Stereo card describing projection lens sleeves
16 Light Tan Presentation box with fitted slots to hold all items

A copy of the December 1953 National Geographic lists this item selling for
$395.00 and included a screen and two pair of Polaroid glasses, however,
this is the one in the presentation box w/Macro attachment.

I know Reel 3-D has the photocopy of the manual available.  I would like to
obtain an original from anyone with a loose or additional copy.  I am
awaiting information from another online contact about the BOLEX MOVIE GUIDE
which was published in 1953.  This 127 page softbound book described the
production involved in using the stereo system including charts.   Any
additional comments from all are welcome.
