Sorry I can't help you with your other interesting questions, but
can mention that since Toy Story was completely computer generated,
unlike traditional cartoons which rely on a mixture of human/computer
drawings, it was a "simple" matter to generate a 3-D pair.
What I found more impressive is how the hand drawn cartoons that are
converted from 2-D to 3-D are often times not simple layering but
have alot more depth information. There are alot of fine examples
of this (that is not to say there aren't bad examples of just simple
shifting :-( ) An example of this can be found at,

This was from an old tintin reel that I had converted last year to
digital.
