It was the era.  I've even seen statements like this in JOSA (Journal of
the Optical Society of America, if I recall correctly) during that time 
period.  So everyone had it wrong at that time.  McKay was probably just 
echoing someone else.  Authors can't check every single thing; they have 
to take some things for granted if they've been accepted all around.
