Neither do I.  That's why I asked the question.

That's a very good suggestion!  Hope the numerous editors reading
photo-3d are taking notes.

PS. Just to clarify a few things:  My suggestions to editors were aimed
to smaller publications... (I did not have the Editor of Stereo World in
mind when I wrote that stuff...) I don't believe that historians have
more free time (never said that either), gentle reminders is the kind
of pressure I had in mind too... and Alan Lewis is darn right about
the web and the internet!
