P3D V-M Personal fogging

Jeremy,
The problem you describe is common, especially when shooting outdoors.
As you suspect, it *is* caused by light leaking through the shutter
blades, and this usually happens only while the lens assembly is at
the bottom of its shuttle (dial set to "A"; when set to "B" the lens
assembly itself tends to block the leakage).
It helps to advance the shutter immediately after taking each shot, as
opposed to just before taking the next shot.
To fix this, cut a strip of black plastic garbage bag material and glue
one edge to the top of the lens assembly and the other edge to the top
of the shuttle assembly -- do this so that the glued strip will fold in
an "S" shape as the lens assembly is raised. Presto! You've got an
internal collapsible curtain which blocks the leaking light. This needs
to be done only on the left lens.
This is far easier to do with the front cover off, so it might as well be
done when replacing the shutter pad...
