Most of the time side-by-side. I have only tried bottom to bottom
once. I shoot slides and mount them in standard stereo mounts so
there are some disadvantages of the portrait-oriented chips.
But if you want to get the minimum lens' spacing then this is
the way to go.

You could, but, as you guessed, it is not advised. The cameras
should be oriented like our eyes. Those who view the world with
heads tilted 90 degrees would natually rotate the cameras by
90 degrees too. But most of us keep our heads straight, hence...
