P3D Address for Kodalus, Fair Lawn, NJ?

Can anyone send me the address of the Kodalux processing center in Fair
Lawn, NJ?  (The one that does Realist mounting.)  I don't have any pre-paid
mailers, and don't have the address handy.

My notes say that it costs $12.08 for processing a mounting a 36 exposure
(29 Realist slide pairs) roll.  Is that correct?
