In the ACM Journal of Graphics Tools, v3n3 (1998!), the first paper
is:

    Bailey, Michael and Dru Clark, _Using ChromaDepth to Obtain
    Inexpensive Single-image Stereovision for Scientific
    Visualization. 

The issue includes a ChromaDepth viewer and a bunch of images,
including the cover.
