P3D Maggie by Martin Simon

Hello everyone,
This is my first time in here.
Does anyone know how was 'Maggie' (by Martin Simon) created? ["Stereo
World" Nov/Dec 1995]. If I wanted to make something similar to that I
would convert the 2D image (the background) into 3D by offsetting the
red channel in Photoshop. But I don't think it was done this way. Any
ideas?
