I believe this is answered in DrT's book... you have a copy,
right? :-)
To be sure that the first exposure is not ruined you have to
advance the film 3 times. (Note: you don't have to cock and
fire the shutter, just advance the film). 2/3 of the times
advancing 2 times will be OK too.
Why this inconsistency? Because when you load the film as you
described (which is the method I follow and recommend) you have
no control over the position of the sprocket wheel which
controls film advance. Official Realist instructions use a
"white dot" and recommend a specific setting of this wheel,
which guarantees that the first frame is not fogged.
Today film is cheap, so why complicate things? If you want to
make sure that you don't miss the first shot, advance 3 times.
