I just took my own advice and looked in Ferwerda and saw another
style of stereo camera which could be the one being used by Mark 
Blum.  The "Nils Melior Stereo Camera" in Ferwerda is a camera
with a single lens which has two apertures.  Since the entrance
is the center of perspective, this arrangement gives you two 
perspectives which is what we need for stereo.  I think Burder
makes a camera like this.
