Go to

and then look for book PM06.  Best $50 you'll ever spend.  (IMHO)

Center of perspective is explained in the first few pages.  Later on,
the various types of formulas for depth of field are derived.

Thanks for the equations used in the web dof calculator.  I'm pressed
for time right now but will look at them later to see if I can figure
out what this negligible discrepancy stems from.
