No.

Toss it.

All changing bags are not IR opaque. Assuming that you did not load
your film in the bright sunlight and fall asleep during the procedure,
the effects you mention are too severe for this to be the complete
explanation.

Perhaps. How is your other film working. If it is okay, then the IR
should be okay.

No. At worst it will bleed over into the other images.

I am assuming that the frames are dark, not the entire negative. If the
entire negative material is dark then you have a leaking problem
somewhere that would be seen with other film.
How did you expose the film? I expose HIE at EI250 metering through a
#25 filter. How did you develop the film? There are many
possibilities, but most problems would be seen with other films also.
