Ernest, I've had the same problem, and I believe I know what it is.
Are you using stop bath (acetic acid)? With D-76 and Rodinal (my favorite
for HIE) stop bath apparently causes small effervescent bubbles on the
emulsion, which lift it from the backing and cause those permanent "spots."
I simply avoid stop bath entirely, substituting a quick water rinse instead
to make my fixer last a little longer.
By the way, I use AGFA Rodinal 1+50 at 68 degrees F for about 16 minutes.
Gives great tonal range!
