When using an R72 filter, I take a reading with a #25 filter first.  I
then add 2-5 stops depending upon the scene at hand.

However, I think that you contradicted yourself by referring to it as
"blank" and "opaque".  If the film was clear, then you grossly
underexposed.  I'm guessing that this was not the case, as even a five
stop underexposure will yield you something.

If the film was completely black then it was overexposed - again, even a
five stop overexposure would yield some kind of image, albeit bulletproof.
If the blackness extended throughout the film (outside of the frames) then
the film probably got exposed to light outside of the cannister.
