fwiw, never had this problem 20 years ago. D-76, indicator stop, fixer hasn't changed since then.
don't see it on other B&W. now there is a different problem of reticulation and air bubbles which 
could show up as a development problem, but that's not being discussed.

I agree with George.  Not the best book to follow to the letter.

Anway, I have only had this problem a couple of times over the last 6 years
or so, but I never use a acid stopbath (for different reasons) and have
always just used water.  So I can assure you water will not cure your
problem.  I believe it is much more likely to be a manufaturing fault.  When
I looked at the last sheet that had it on under a microscope at  x100 you
could see that the emulsion was either just not their or much thinner.  I
have always wondered if it is caused by bubbles in the emulsion during the
coating process.
