  It sounds like you have too
much on-film deviation. If you
overlay the two film chips and
line up the nearest object (blade
of grass or whatever). Then the
furthest object should be separated
by no more than 3mm or so.  If
they are more than 3mm apart you
have too much on-film deviation.

  Those near and far distances
in your table only apply if there
is _nothing_ closer or further
in the scene. Though an out of
focus sky or other featureless
background is okay.

  Send your film chips to Tom
or someone else for a diagnosis.
You could send them to me too
but I'm in Canada so it would
be slower.
