  Toe out would definitely hurt the
eyes.

  What kind of cable release does
it need?  It doesn't perhaps have
a threaded ring around the shutter
button to take a Leica adapter like
I use on my Ricoh 225 TLRs?

  As far as your camera rotating
due to a small tripod screw spot.
I had the same problem with my fisheye
66 camera. The tripod mount area is
a 0.625" circle of metal that sticks
about 3mm below the camera body.
So I cut out a square of thin plywood
and drilled a .625" hole in it and
attached the plywood to the camera with
some screws. Now the tripod head grips
on a much larger area.

  For your Kiev perhaps you could do the
same thing but without screwing it to
your camera. The friction of the wood
to the camera body might be more than
enough to prevent rotation.
