Qualex has had Kodalux for some time. When Kodak recently
purchased Qualex, Kodalux went away and Kodak Premium 
processing took its place. At least that is what happened in
Southern California. I got tired of holding my breath when I
would send in slides. I have received slide film I shot with my
viewmaster camera mounted, even though I wrote 'do not cut',
'do not mount' all over the envelope and roll of film. I have also
received it back half mounted and half uncut!

Ever since Gabriel posted that he takes his film to Costco, I 
have used them and have been very satisfied.
