Trouble with Kodak Premium Processing

Has anyone else had bad experiences with Kodak Premium Processing? 
They've been advertising it like crazy, but if my experiences are
anything to go by, Kodak is completely unable to deliver consistently
the features they advertise.

I took two rolls of film in to be processed with Kodak Premium, and the
pictures came back without index prints.  There was a note saying that
due to equipment failure, they were unable to produce index prints, and
"under the circumstances" they felt it was better to get my pictures
back to me as soon as possible.  (Translation:  the lab didn't want to
get backed up.)  They included coupons for "free" index prints, so I
sent the negatives back to get what I should have gotten in the first
place.  When they came back, however, the negatives no longer had the
protective sleeves that are an advertised feature of Kodak Premium
Processing.

On my next roll of film, Kodak mistakenly processed it using standard
processing, despite the fact that "Kodak Premium" was clearly indicated
in two places on the envelope.  I sent the pictures back to be redone as
Kodak Premium, but they came back with only Premium reprints -- no index
print or protective sleeves.  I sent them back _again_ with clear
instructions as to what I wanted, and they came back with an index print
but still no protective sleeves.

What is going on with Kodak?  Do they not require their employees to be
able to read?
