Unfortunatly yes, this can happen, although devices that can cause a
"field-flip" so to speak, are few.  I've run into at least one TBC (Time
Base Corector) that flips the stereo image, and one of the Sony DVC
editing decks has been reported to do "fields-flips".  ...Apparently
with this certain DVC deck, when the video is converted to digital,
you have a 50/50 chance of getting the stereo field order correct.
My strong suggestion is to run a 3-D stereo test pattern through
your various equipment to look for any field-flipping gremlins.
