P3D NSA'98 advertising question - help?

To whom it may concern:
I just realized that my deadline for the advertising in the NSA Program
Guide is upon us. I have a few technical questions that need to be
answered rather quickly, if I'm to meet the deadline. There is no contact
info on the NSA rate sheet other than a street address. Would the person
handling advertising, or anyone who knows that person, please help me with
these questions?
The rate sheet specifies that the ad be submitted "camera ready." That can
mean different things... Please only answer if you are actually in charge
of, or have direct contact with the printer producing the NSA Program.
These are not theoretical (rhetorical?) questions submitted for general
discussions of printing technology, or other guesswork:
1. Does the ad have to be actual size, or can the printer "shoot it down"
to the correct size?
2. What is the preferred (maximum) screen frequency on-press for
half-toned images?
3. Might the printer prefer that I submit a continuous tone original, that
they will half-tone for me to their specifications? Is there an added cost
in that case?
