P3D Phantograms

Wow.. how did you shoot this and introduce the correct distortion factor. I have pondered
doing this myself (although hadn't considered life-size.. great idea) with a field camera
that a friend has. I assume that adjusting the film plane to be parallel with the floor 
while holding the lens at 45 degrees to the subject would introduce the correct
distortion... or am I wrong ? 

I would love to see one of these images done with polarised inks....(stereojet?)

I don't suppose theres any chance of a digital "sampler" that we could see/printout
(not life size of course..)
