Funny, that's exactly the opposite of my experience.  The ONE time that I saw 
a consisitent problem like this was in a minilab when the printer (the human 
operating the machine) was in such a hurry that he pushed the film along 
while the second print was still being exposed.  (That was back before 
minilab equipment had motorised film transport [a real dinosaur: Copal 100 
EP-2 with running water wash, state of the art circa 1982].)

I've also seen this (but very rarely, and again on older equipment (Noritsu 
802/901)) when the operator forgot to program the repeat key, and compenated 
by pushing the print key twice, manually.  Apparently the film gate would 
"bounce" a little between exposures, causing something like this.

Dave and Mark make several very good points.  I only wish the digest had 
arrived before I posted my own response.  How about a little quicker, next 
time, eh guys? {;)
