There are really at least 3 possibilities in Kodak's terminology:

Pro Photo CD (6 resolutions, highest is 72 MB per file)
Photo CD (5 resolutions, highest is 18 MB per file)
Picture CD

Several years ago, when Kodak was the only supplier for Photo CD, I sent a
stack of mounted slides in for Photo CD scanning.  Most were non-stereo,
but I included some 5p and 7p frames mounted individually in Gepe 2" x 2"
glass mounts and they got scanned along with the rest.  These were all
scanned in their mounts.  I was pleased with all the results.  Slides in
cardboard mounts with curved inside corners showed a dark area at each
corner, which would be almost unavoidable.  As has been noted, it wasn't
cheap - over $2.00 per frame.

This order was sent to Kodak by my local photo retailer which deals with
both pros and amateurs and knows what it's doing.

As I remember what's been written in the photo magazines, one of the
distinctions between PhotoCD and PictureCD is that Kodak will accept
individual frames for scanning to PhotoCD, but Picture CD is only for whole
rolls scanned at the time of processing.  It's probably all there on the
web site.  Other suppliers, who are now in the business, may be doing it
their own way.
