T3D Where?

O.K., I'm STILL confused about where to take my base measurement (i.e. 
the camera) from for macro stereo: the film plane, the middle of the 
lens (whatever the technical term is), or the end of the lens for 
[film/camera/lens] to subject distance. I have been repeatedly told all 
three, which doesn't make sense to me, which is probably why I don't 
exert any effort in using formulas. Forgive me if this has been stated 
plainly already; I couldn't find it.
