Just a quick thought.  If you are using a lens with a large number of
glass-air boundaries. as for example a zoom or a very fast lens,
it is possible to see flare far more quickly than in a simple
but well corrected lens like, for example, a Tessar.

Because you are operating at or outside the extreme limits of the 
coatings' design range, they often become ineffective, and no
better or even worse than a single coating.  I have found my single
coated Tessars and Xenars to be quite flare-free, but I would really
wonder about a lens with 6 or 8 internal glass-air surfaces or more.
