My friend, Bob Munn of Depthography, Inc. does
lenticulars.  You can call him at (212) 679-8101.
He's here in New York City.  Depthography's
address is 122 E. 27th Street, New York, NY,
10016, where he has a gallery, called The
Virtual Image, of 3D and animated lenticular art. 
Depthography also does high-volume lenticular
production for advertising, etc.  I'm sure
they can beat Kodak's prices.
