P3D Realist operator error?

I'm disinclined to blame the following problem on my 3.5 Realist (which
has never before or since done any violence to film). Nonetheless, I
solicit the opinions of others who may have "been there; done that."
The first of two rolls of Sensia 100 I shot over the weekend came
back from the lab with one tear between two sprocket holes about mid-roll.
As expected, the following two exposures had one frame each about one perf
out of register, and the balance of the roll was OK.
Although I can't recall any peculiarities advancing this film, my best
guess is that I may have started turning the advance knob before the
advance button was fully depressed (thereby pulling the film a short
distance through the still-locked sprocket). Obviously, I'd rather chalk
this up to "operator error" than to a camera problem.
Anyone care to weigh in on this?
