P3D Making WM reels

I am interested in producing a VM reel and have a 
number of questions. I know some of the peolpe on this
list have produced reels in the past. Any insights 
would be appreciated.

What is the general proceedure for have a reel manufactured.
What is the cost? How long does it take?
What is the best film and format format for the original images?
I usually shoot color print film in 5p format. Can I use those 
images?
