As far as I know twin cable releases were (and maybe are) used in two
situations :

1) Use of a (SLR) camera with bellows
2) Use of a (SLR) camera with tele (100-135mm) - reversing ring - standard lens
(50-55mm)

In both cases there is no 'coupling' (possible) between the camera and the front
 lens
Twin cable releases were never used (IMHO) for synchronisation although
you can twiddle with them (if they are adjustable) to get a degree of sync for
stereo
work.

Their  purpose was (after thoughtfully adjustment) to first close down the
aperture of
the front lens (by means of the first release) and then make the exposure (by
means
of the second release).
