The quote says it clearly, if this was in fact on the form, which I've
never seen. The key phrase is "with your payment". Trust me. I'm an
English major.  If you didn't send the money, you screwed the pooch. Tough
luck-- you blew it. Be responsible for your actions. If you did send the
money and didn't get a table then someone at NSA needs to make it right.
Can we call this to done and get on with life?
