I had agitation problems when I filled the tank too high. Better to leave
some room for the developer to slosh around. I might dev 3 rolls in a 4 roll
tank, leaving room for air space. "Sufficient" agitation is very important.

Make sure your hypo is fresh, as is your PermaWash.

On another note, my friend was once using selenium toner instead of
photoflow for his final rinse, 'cause the bottles looked alike, and this
left stains....
