Uneven development?

I have been developing HIE for quite some time - usually using a 4 roll tank. 
 Every so often on a number of frames, I get what at first blush looks like a 
well balanced developed neg, but then appears as a different gradation 
cascading down the frame.  It almost looks like a stain.  I use a distilled 
water pre-soak, and only agitate once a minute.  Any ideas on how to prevent.
