The place is Dale. They always advertise in "Popular Photography", and 
they do adequate work. Seattle Filmworks also makes slides from negative
film, but they aren't set up to leave the slides unmounted. I once used
my Nimslo with print film, sent it to Dale, mounted the slides as stereo
pairs (and saved the middle chips, just in case I wanted 2-D slides),
then sent the negatives to Nimstec for lenticular prints.
