I seem to remember that I got a sample once from Cygnus Graphics in Flagstaff, 
the makers of the anaglyph/3D topo map of the Grand Canyon and other things. I don't know
if they produced it themselves, though. This wasn't attached at the edge of a fold, though -
it was more similar to a business envelope with the address window (but which you could
tear apart to make lightweight red/blue (or was it green?) specs).
